Cuts or injuries caused by skate blades
Muscle strains, sprains, fractures and other musculoskeletal injuries
Head, neck or spinal injuries

PARTICIPANT RESPONSIBILITY

I confirm that the participant is physically capable of participating in power skating and related training activities.

I agree to advise the coach before participation of any injury, medical restriction or physical limitation that could reasonably affect safe participation.

I understand that it is the participant's responsibility to:

Follow coaching instructions
Follow all arena and facility rules
Wear appropriate and properly fitted equipment
Conduct themselves safely and respectfully
Stop participating and advise the coach if they become injured, unwell, dizzy or otherwise unable to safely continue

I understand that the coach may modify or stop an activity if there is a safety concern.

PROTECTIVE EQUIPMENT

I understand that skating and hockey activities require appropriate protective equipment.

Where required by the facility, program or coach, the participant must wear a properly fitted CSA-approved hockey helmet and any other required protective equipment.

The participant and/or parent or guardian is responsible for ensuring that personal equipment is properly fitted, maintained and suitable for use.

ASSUMPTION OF RISK

I freely and voluntarily assume the ordinary and inherent risks associated with participation in True North Edge power skating lessons, training sessions and related activities.

I understand that participation is voluntary and that I may choose not to participate in an activity that I believe is unsafe or inappropriate for the participant.

RELEASE OF LIABILITY – ADULT PARTICIPANTS

This section applies to participants who are 18 years of age or older.

In consideration of being permitted to participate in True North Edge activities, I agree, to the fullest extent permitted by applicable law, to release and hold harmless True North Edge, Sabrina Coomber, and their respective representatives from claims arising from the ordinary and inherent risks of participation that I have knowingly and voluntarily accepted.

Nothing in this agreement is intended to exclude or limit liability that cannot legally be excluded or limited under Ontario law.

PARTICIPANTS UNDER 18

For a participant under the age of 18, I confirm that I am the participant's parent or legal guardian and have authority to consent to their participation.

I acknowledge that skating and athletic training involve inherent risks and give permission for the minor participant named above to participate in True North Edge activities.

I understand that this acknowledgement and consent does not waive any rights that cannot legally be waived on behalf of a minor.

EMERGENCY MEDICAL AUTHORIZATION

In the event of an injury or medical emergency, I authorize True North Edge and/or the coach to contact emergency medical services when reasonably considered necessary.

If I am the parent or guardian of a minor participant and cannot be reached, I authorize reasonable emergency assistance to be obtained for the participant.

I understand that True North Edge and the coach are not medical professionals and are not responsible for diagnosing or treating medical conditions.

Any costs associated with ambulance transportation, medical treatment or other emergency services remain the responsibility of the participant and/or their parent or guardian.

CONCUSSION & HEAD INJURY

I understand that skating and hockey activities carry a risk of concussion and other head injuries.

If the participant experiences or displays symptoms consistent with a possible concussion or significant head injury, the coach may immediately remove the participant from training and require medical clearance or appropriate return-to-sport measures before participation resumes.

Participants and parents/guardians agree to promptly disclose suspected concussions or other injuries that could affect safe participation.

FACILITY & THIRD-PARTY ACTIVITIES

I understand that True North Edge may provide coaching at arenas, municipal facilities or other locations that are not owned or operated by True North Edge.

The participant remains subject to the policies, rules, admission requirements and conditions established by the applicable facility.

Any applicable arena admission, membership or ice fees are separate from coaching fees unless specifically stated otherwise.
